He needs transportation to and from work Monday through Friday from 5:00 a.m. to 9:00 a.m. He anticipates returning to 6-hour workdays in the future, at which time he will also need pickup at 11:00 a.m. He has worked for the same company since 1991 and takes great pride in maintaining his employment. Reliable transportation and clear communication, especially if there are any delays, are very important to him. Person #737: Mansfield Man with Level 1 Waiver Wants an HPC Provider

(2/27/26) A 27-year-old Mansfield man is seeking an HPC and HPC Transportation provider for 5 hours per week. The schedule is flexible to allow for community outings and doctor’s appointments, which occur 1–5 times per month. There are no known or likely risks, and he is able to be alone independently. He would benefit from support with monitoring his physical and mental health, scheduling appointments, and ensuring Professional Follow-Up forms are obtained from medical providers and submitted monthly to the SSA (with staff notes attached if handwriting is unclear). He is very independent but does not drive, so he needs reliable transportation for appointments, shopping, and community activities. He would also like support building friendships, attending sporting events, and becoming more involved in the community. He enjoys playing PS5 with his brothers, spending time with family (including their dogs and cats), drawing, reading, cooking new recipes, riding his electric bike, and keeping a consistent routine. He has a great sense of humor and is enjoyable to be around. He prefers patient, respectful providers who give him space and avoid being overly directive. (2/27/26) A man in his 50s is seeking a provider for transportation to and from his home in Mansfield to his workplace in Bellville. He typically works four days per week, with a schedule that changes weekly. His shifts are usually 11:00 a.m.–3:00 p.m. or 3:00 p.m.–9:00 p.m., and he receives his schedule one week in advance to share with his provider. He communicates well and will notify his provider if he is getting off early or if his schedule changes. He does struggle with his concept of time, so it is especially helpful when his ride calls or texts to let him know they are on the way. A 28-year-old female is seeking a provider to assist with Non-Medical Transportation (NMT) to her Adult Day Services (ADS), volunteer position, and community job. She needs transportation to ADS on Mondays at 9:00 a.m. with pickup at 3:00 p.m. On Wednesdays, she volunteers at a local agency from 11:00 a.m. to 2:00 p.m. She also works in the community two to three days per week. Her typical schedule is Saturdays from 9:00 a.m. to 2:00 p.m. and Sundays from 12:00 p.m. to 4:00 p.m., with a summer schedule of Friday, Saturday, and Sunday from 9:00 a.m. to 2:00 p.m. She would benefit from a provider who is consistent and kind, includes her in decisions about her life, speaks slowly and clearly while facing her so she can hear and understand, and provides advance notice of outings or schedule changes so she can be prepared. Person #734: Provider needed for HPC Services for Mansfield Child with Local Funding ASAP

(2/27/26) A friendly 14-year-old who needs support with daily personal care. This includes help with changing briefs, dressing and undressing, preparing meals and snacks, and some assistance with feeding, such as placing food on utensils and helping guide food to his mouth. He uses a power wheelchair and needs help reaching items and participating in activities in areas of the home that are not wheelchair accessible, as well as assistance with transfers. Support is needed from 7:30 a.m. to 12:00 p.m. for the next 3–4 weeks, and then from 2:30 p.m. to 5:00 p.m. He does not have alone time at home or in the community, auditory supervision is required at home, and close, constant supervision is needed in the community. He enjoys going for walks in warm weather, playing board games like Uno, and being read to. It is important that staff listen to him and engage with him, as he enjoys interaction even though he may repeat himself or be difficult to understand at times. Person #733: Mansfield Man wants an IO Waiver provider of Shared Living Services

(2/24/26)A 56-year-old man is seeking a shared living provider and requires 24/7 support. He needs full assistance with transfers (including Hoyer lift use), meal preparation, personal care, bathing, dressing, cutting food, cleaning, and daily routines. While he requires comprehensive care, he may choose to have up to four hours of alone time. Known risks include UTIs, bed sores, and asthma attacks, so attentive health monitoring is essential. He prefers providers who are caring, treat him like family, enjoy showing affection (he likes hugs), and clearly explain things while allowing him to ask questions. Due to 50% hearing loss in both ears, it is important that staff face him when speaking. He values provider input in decision-making and feels safest in a home environment, with access to a watch or alert system if staff are out of the room. He would like support attending medical appointments, getting involved in the community, and maintaining a circle of accepting staff and friends. (2/23/26) - The family of a happy 10-year-old girl is looking for a FEMALE provider of HPC services. The hours primarily needed would be after school & on weekends.  This child has cerebral palsy and needs full assistance with her personal care, repositioning, and supervision while eating.  She can hand feed herself but has to be watched as she will eat fast and may choke.  She also needs assistance with changing, moving, and dressing herself.  She is most happy when she is with her family or with her horse.  She loves going to Raemelton Horse Farm, loves dancing, and loves anything involving animals. Person #729: Mansfield Woman wants an HPC & NMT Provider of Level One Waiver Services

(2/12/26) An 18-year-old woman from Mansfield, who is homeschooled and taking college courses, seeks a female provider for HPC services, & Non-Medical Transportation. She needs a non-medical transportation provider 2–4 times per week, with work hours from 10 a.m. to 2 p.m. Hours will vary weekly based on her schedule and will be coordinated with her and her parents. She has a service dog, (a Standard Poodle), who accompanies her on outings; the provider must be comfortable transporting the dog (a seat cover will be provided). She enjoys being active, with interests in hiking at Mohican, visiting Cedar Point, exploring Columbus (zoos, parks), and spending time at local coffee shops for schoolwork.  Person #725: Provider needed for Respite Services for Shelby Woman with IO Waiver

(2/11/26) A 52-year-old woman is seeking residential respite one weekend per month, as well as April 7–14, due to the temporary absence of her caregiver. She attends an adult day program Monday through Friday from 8:00 a.m. to 3:00 p.m. and has 15 minutes of unstaffed time. She requires supervision to ensure safety, frequent reminders, encouragement, and support with decision-making. Consistency, patience, repetition, and advocacy for medical needs are essential. She enjoys joking, being helpful, animals, music, Hallmark movies, game shows, playing games, walking, and spending time outdoors. It is important that she has reminders, positive reinforcement, an outlet to express her feelings, accommodations for low vision, and support in recognizing and addressing medical concerns. He needs someone to stay with him for 1-6 days, assist with NMT transportation, and drive him to/from ADS. (2/2/26) A 20-year-old man with local funding is seeking a provider for up to 8 hours of HPC per month. Independent Providers who are interested in working with the individual will need to apply through the NECIC process in order to receive payment. It is a different process & pay. He would like support getting out of his home and into the community at least one day per week, as he currently spends much of his time indoors due to limited transportation and support. He enjoys walks, hikes, and spending time outdoors (particularly in cooler weather) and prefers to avoid large crowds. He has no known or anticipated risks and has an open schedule, allowing flexibility in days and hours. HPC transportation will be required. (1/22/26) A 30-year-old woman in Mansfield seeking Residential Respite and HPC services. Residential Respite is needed several times per week for up to 12 hours total, and HPC may be needed 1–2 days per week for up to 16 hours per day. Supports include cutting food, assisting with drinking, bathing, toileting, medication administration (Med Cert 1 required), ensuring safety, and accompanying her in the community. She works best with patient, kind, gentle, soft-spoken, non-smoking providers and prefers younger female staff. She does not respond well to people who are bossy, loud, unrealistic, or unclean, and becomes frustrated when her mother is not home, during arguments or loud environments, when her belongings are touched without permission, or when her bed is not made. She enjoys music, library visits, and movies. Family is very important to her, and her mother, is her guardian and primary decision-maker. Due to seizures and limited vision, he is a fall risk and requires support with mobility, especially in unfamiliar environments and on uneven terrain. He should remain within sight at all times. The provider should help him explore new activities, assist with stairs, allow time for him to respond to questions, and involve his family, when possible, as their presence helps him feel comfortable. (12/10/2025) 10-year-old girl is seeking an HPC provider for 15 hours weekly, Monday–Friday, from 7:30–9:30 a.m. and 3:30–4:30 p.m. She requires 24/7 supervision and support with her morning routine and after-school supervision until her caregiver returns. She enjoys being outdoors, swimming, hiking, parks, and watching sensory videos, and she likes action figures such as Spiderman, Paw Patrol, and PJ Masks. She prefers healthy snacks like fruits and vegetables and enjoys drinking water, with gentle monitoring as needed. She is active, curious, and enjoys exploring, and benefits from guidance to stay safe and calm during high-energy moments. She completes many tasks independently but may need verbal cues or reminders to use her AAC device.
